CVHS Athletic Department for a list of forms required to participate in the Crescent Valley High
School Football Program.
These forms include the:
Forms and fees ($150) must be submitted by the end of the
school year for the prior season, or by the first day of August practice. If
not submitted until the August practice, the student is not able to
participate in summer workouts.
